Considerable progress has been made towards the achievement of a single national monitoring and evalua-tion system in Baylor supported regions. District report-ing rates have greatly improved to 95%-100% with more district teams having adequate capacity to collect, anal-yse, generate reports and utilise their data for decision making. During the year, Baylor continued to support over 100 sites to use Electronic Medical records Sys-tems (OPENMRS) which has led to a great improvement in data quality and use. All districts and facilities were supported to conduct joint review and planning meet-ings in which action plans were generated to address identified gaps. These meetings attracted a number of participants including health facility staff, district top leadership, ministry of health officials, cultural leaders and implementing partners.
